Otoplasty, Ear Pinning and Ear Lobe Surgery

Otoplasty is the cosmetic correction of the ears (ear pinning) or the ear lobes, often due to an injury, birth defect, or intentional alterations such as expansion of the lobes.

For some ear surgery patients, this procedure is considered because they are unhappy with the appearance of how far their ears protrude from their face. Children and teens comprise the largest group of our patients who undergo "pinning back" of the ears.

What To Expect During Surgery

  • Ear surgery on children or teens is usually performed under general anesthesia with an anesthesiologist monitoring the patient at every moment.
  • Adults can opt to have the ear numbed during the procedure with local anesthetic.
  • For injuries to the ear that have resulted in disfigurement, proper steps will be made by our ear surgeon to bring the ear back to as normal an appearance as possible.
  • Sometimes skin grafts or cartilage from another area can be transplanted to simulate a normal ear shape.
  • For earlobe surgery, our surgeon will assess your specific situation. No specific corrective steps can be outlined because it will be up to the discretion of the ear surgeon.
  • For ear pinning, an incision is made in the fold of the ear where the scar will be least noticeable. Our ear surgeon will skillfully remove any extra cartilage that is causing it to protrude. Then, the cartilage end is re-joined to the fold of the ear so that it can heal in its new position.
  • Otoplasty is one of the few cosmetic surgery procedures where the incision can be so well-hidden that it is virtually invisible!


The patient will possibly feel dizzy from the anesthesia after the surgery. It is important for there to be someone present to help the patient move around, get food and to help keep them hydrated. Painkillers will be prescribed to keep the patient comfortable and help them rest better.

There will be some post-operative swelling which can be greatly reduced by keeping the head elevated above the heart with a few pillows. Due to the anesthesia, the patient may want to stick to bland, soft foods that are easy to digest for the first day or two.

We generally prescribe antibiotics for the first week to ten days after otoplasty surgery to lessen the risk of infection at the surgical sites. It is important to precisely follow your Post-Operative Instructions sheet to avoid complications.

Compression Garment And Recovery

Our ear surgeon will require that you wear a compression garment for the first week or so to keep down the swelling.  The sutures will be removed from the outside of the ear after a couple of weeks. For the 6 weeks or so after the compression garment has been removed you will be asked to wear a soft elastic ear band at night to hold the ears in their new positions.

During the healing phase of the incision, you will be asked to gently cleanse the site with a Q-Tip and then apply an antibiotic ointment to help keep the incision free of bacteria and help it heal more quickly.
